This form Incident Report Form should be completed as soon  as possible after an incident involving injury or property damage, or after a near miss.

You need JavaScript enabled to view it.  (within 10 days if possible), attaching any relevant supplementary information. Ramblers will advise when it is received  and  copy it to the EBR co-ordinator, or area/ group secretary for their information.
